247 Care provides the highest quality in-home Elder Care to clients residing in Toronto  GTA and Durham Region .
247 Care specializes in providing services to the elderly. We offer assistance that help elders stay independent and safe in their own homes. FREE In - home assessment conducted by one of our qualified staff.

247 CARE is committed to supporting the needs of our community’s Elders aged 60 to 64; and Elderly, referring to people aged 65 or above living independently or with family. 247 CARE formulates an Individualized Care Plan  (ICP) together with the Client and family members or representatives as appropriate. The assessed needs of the Client, e.g. physical, mobility, dietary, emotional, social, etc. and how these needs are to be met are clearly stated in the Individualized Care Plan (ICP).

247 CARE is most commonly used by Clients who are assessed at no to mild level of need but require only personal care and home-making services.

 

247 CARE provides, arranges for, or purchases the following direct services required to enhance the Clients' quality of life in their home environment:

  • Personal care;

  • Simple nursing care ;

  • companionship

  • running errands

  • General household or domestic duties;

  • Transportation and Escort;

  • Home respite service;

  • Health/Safety education and review;

  • Purchase and delivery of daily necessities;

  • Provision of meals;

  • Laundry service;

  • Other support requested by the family, such as: child-minding, shopping, errands, etc.

247 CARE is also used by Clients who are assessed at moderate to severe level of need requiring a more comprehensive package of services.  We address the Clients' health concerns and corresponding needs by applying a multi-disciplinary approach including nursing care (non-medical), nutritional care, personal care, rehabilitative & social work services.  

247 CARE provides a well-coordinated package of home-based and community-based services tailor-made for the individual Client. The management of clinical issues includes the following:

  •  Management of falls;

  •  Prevention and management of accidents;

  •  Management of skin integrity;

  •  Supervision of medications; 

  •  Management of depression;

  •  Management of cognitive impairment;

  •  Maintenance and restorative rehabilitation.

To address the above clinical issues and to meet the needs of individual Clients, 247 CARE provides, arranges for, or purchases the following services on behalf of the Client:
  • Home respite, day respite, and residential respite service;

  • 24-hours a day, 7-days a week emergency support;

  • Environmental risk assessment and home modifications;

  • Home-making service e.g. household cleaning, changing of bed  sheets and pillowcases, purchase of necessities, and laundry;

  • Provision of meals;

  • Transportation and escort service;

  • Other support requested by the family, such as: child-minding, shopping,  errands, etc.

In the care of Clients with dementia, 247 CARE provides staff with special training in communicating and dealing with Clients with mood and behavioral symptoms associated with dementia such as poor temper, unrealistic fears, repetitive complaints, agitation, wandering, and aggression.
